Why is the free hotel breakfast a “continental breakfast”?

What makes a breakfast “continental”? Does coffee and donuts make something continental? Is it only offered in North America? How did this tradition get its name? Anyone know?

    The continent typically serve cold breakfast of cereal cheese and croissant as selfservice buffet.
